Send in (upload) your completed submissions below. Only PDF files will be accepted. Name the file using the format “PCAI_Option [#]_[Full Name or Organization Name]_[YYYYMMDD].
A green confirmation box should appear within the form upon successful upload. Please keep watch after pressing “Submit” and do not click away or refresh!
NOTE: We will not be posting the contact information that you submit within the form fields, EXCEPT FOR “Individual or Organization Name, or Description of group that did micro-consultation” if it was not included in the submission document itself. So the submissions will not be anonymous. Any information within the submission PDF itself, such as names, will be shared as we are posting all submissions as-is.
DEADLINE: Extended to Monday, March 23, 2026 at 8:00pm PST / 11:00pm EST. In fairness to those who meet this and to keep the process organized, this portal will be taken down once the deadline passes.
